PATIENTS RESPONSIBILITIES

  1. To be completely honest in providing past history of illnesses, personal habits, presents problems and family history.
  2. To provide information about allergies, problems during previous medical treatment, outside opinions and outside medical records if any.
  3. Patients should respect parking rules, visitors’ timings, hospital rules, hospital rules, dignity of hospital staff and other patients.
  4. Patient should participate in decision making, regular communication with Doctors and make an honest attempt to complete entire treatment plan.
  5. To refrain from violent behavior towards hospital staff, hospital property & other patients.
  6. Patient should provide accurate demographic information, e.g. full name, age, address etc.

Instructions

You are being directed to CPGRAM website. You would be asked to Register with CPGRAM if not already registered. Registered users can login directly using their CPGRAM user ID and password.

  • 1. Login into CPGRAM
  • 2. In section "Grievance Detail" select "Others/Not Listed/Not Known"
  • 3. In the "Ministry/Department" Drop Down Select ESIC
  • 4. In "Subordinate Department/Office" select the Regional Office
  • 5. Provide details of the complaint
